What to Bring to Your Doctor’s Appointment

There are a number of things that our workers’ comp patients can do to make sure they get the most out of their doctor’s appointment at the Orthopaedic & Spine Center of the Rockies.

Please remember that our ability to give you the best, most accurate diagnosis and treatment plan depends on the information you and your doctor can provide us.

Arrive a Few Minutes Early

Please arrive prior to your actual appointment time to complete paperwork.

Our Case Management Department works with you, your insurance company, your doctor, your employer, and your OCR physician by collecting your workers’ compensation information before you have your appointment with us. This includes your claim number, date of injury, workers’ compensation insurance, and information from your claims adjuster.

Please Bring These Items to Your Appointment

Please bring these items with you to your appointment, or have them sent to the Orthopaedic & Spine Center before your appointment:

  • Medical office notes from your previous appointments with your primary physician
  • MRI, CT scans, or X-rays — either on a CD or on film
  • Any report with the results from your MRI, X-ray, or CT scan
  • Any EMG report and results
  • Reports on any injections you have had
  • Reports on any surgeries you have had
